Client Information Forms
We use the HIPAA Forms service on our website to receive and store personal demographic and health information from clients. This service is encrypted except for the first and last name, email address, and phone number, and the information is stored securely on the HIPAA Forms servers. It may be printed and stored securely, but is not ever e-mailed. We also collect membership application forms through a secure file transfer service called sendfilessecurely.com. This service is encrypted. However, if you choose to submit information through these services, Whole Health Discoveries, LLC cannot guarantee that the information will remain confidential. Even though the HIPAA Forms and sendfilessecurely.com services may utilize state of the art encryption methods, firewalls, and/or back-up systems to help secure the information, there is a risk that the electronic communication may be compromised, unsecured, and/or accessed by an unintended third-party. There is never a 100% guarantee information will remain confidential when transmitted electronically.

Website Analytics
WP Statistics is an online service which helps us to understand our website visitors; for example, how visitors find our website, how long they spend on our site, which web pages they are most interested in etc. We use WP Statistics because better understanding how visitors are interacting with our website helps us to improve the content, design and functionality of our site. This allows us to offer a better online experience to our visitors.

WP Statistics does not use cookies, and it only collects basic, non-identifying information about visitors.

Information about your financial situation and medical conditions that you provide to us in writing, via email, via texts, on the phone (including information left on voicemails), contained in or attached to applications and forms, or directly or indirectly given to us, is held in strictest confidence. Limited Right to Use Non-Identifying Personal Information From Testimonials, Letters, and Other Sources: 
Any pictures, stories, letters, testimonials, correspondence, or thank-you notes sent to us become the exclusive property of Whole Health Discoveries, LLC. We reserve the right to use non-identifying information about our clients (those who receive services from or through us) for promotional purposes.

Clients will not be compensated for use of this information, and no identifying information (photos, addresses, phone numbers, contact information, last names, uniquely identifiable names, or uniquely identifiable experiences) will be used without the client’s express advance permission.

You may specifically request that NO information be used whatsoever for promotional purposes, but you must identify any requested restrictions in writing, at info@wholehealthdiscoveries.com. We respect your right to privacy and assure you no identifying information or photos that you send to us will ever be publicly used without your direct consent.

Who we share your data with
Your data is not shared with anyone other than as stated here in this policy.

How long we retain your data
—Client forms will be retained for 7 years after the most recent service date, either in electronic or paper form.
—The most recent 100 page views for each user are kept in WP Statistics.
